Master Plan on Aging Community Needs Assessment Proposal - Request for Qualifications #2022HSD09

The County of Santa Cruz Human Services Department invites proposals for a Master Plan on Aging (MPA) Community Needs Assessment. Collective of Results and Evidence (CORE) Request for Proposal - #HSD1-2021

CORE Investments is a local movement to achieve equitable health and well-being in Santa Cruz County, using a framework that is a collective impact, results-based approach that is responsive to community needs.  CORE Investments also refers to specific City and County funding dedicated to improving equitable well-being through the application of the CORE Framework.  The Santa Cruz County Board of Supervisors and City Council on November 9, 2021, approved the release of an RFP that outlines their intent to collectively award $5.8 million annually for three County/City fiscal years. The application period closed March 4, 2021.

Housing Navigation Services Proposal - Request for Qualifications #2020HSD06

The County of Santa Cruz Human Services Department invites responses for social services consultant(s) or organization(s) to provide Housing Navigation services. The proposed scopes of work for the Home Safe Program within the Adult Protective Services (APS) unit of the Human Services Department (HSD) Adult and Long Term Care (ALTC) division can be found in Attachment A1. The proposed scope of work for the Transitional Age Youth program within the HSD Housing for Health and Family & Children’s Services (FCS) divisions can be found in Attachment A2. Consultant(s) or organization(s) must have familiarity with housing navigation, including landlord engagement and knowledge of Housing Authority programs.
