What information do I need?

Once your application is received, an interview appointment may be necessary and will be completed by a Benefits Representative. It is important to not miss your appointment. You may be asked to provide verification of the information provided during the interview if it cannot be electronically verified or is questionable. Here are some of the items you may be asked to provide:

  • Income
  • Birth/Citizenship
  • Identity
  • Santa Cruz County residency
  • Social Security Number
  • Expenses – including housing and utility costs
  • Disability for any disabled person in the household

If you do not have all the required documents, still attend the scheduled interview. The Benefits Representative completing the interview will give you time to provide any missing verifications. Please discuss any difficulty in obtaining the verification requested with the Benefits Representative, they will help you get the documents if necessary.
