The Human Services Department is dedicated to the well-being of older residents and people with disabilities throughout Santa Cruz County.
The Age Well Santa Cruz County initiative is our county's collaborative process for developing a local Master Plan on Aging. This plan, or "MPA" will be the blueprint we use to help ensure people of all ages and abilities in our county are engaged, valued, and afforded equitable opportunities to thrive as they age, how and where they choose.
MASTER PLAN FOR AGING "CNA" SURVEY
As part of Age Well Santa Cruz County and designing our MPA, the Human Services Department issued a county-wide Community Needs Assessment, or "CNA" Survey on January 8, 2024. The survey collected community feedback focused on aging in the county. By the survey's close on March 31, 2024, over 3,200 residents aged 40 and older across the county had responded. Preliminary findings from the survey were published in September of 2024, and the final CNA survey report was released in August of 2025.

MASTER PLAN FOR AGING "LOCAL PLAYBOOK"
The Master Plan for Aging's Governance Group, under the leadership of the Senior's Council, recently developed a local playbook to serve as a guide for our shared work to enhance the quality of life of county residents as we age by maximizing collective impact and aligning overlapping efforts to best support our community.
