Medi-Cal Health Benefits

How do I apply?

You can apply for Medi-Cal by mail, phone, online, or visit a Human Services Department Application Center.

If you cannot get to the application center, someone who knows your circumstances, and is authorized by you, can apply and be interviewed for you. If you are disabled and cannot get someone to apply for you, you can apply by mail or phone.

  • To apply online visit: www.benefitscalwin.org
  • To request an application by telephone call the Benefits Call Center at 1 (888) 421-8080.
  • To download the Medi-Cal application from the Department of Health Care Services, visit their website at http://www.dhcs.ca.gov/.

When you apply, you will be asked about the following:

  • Residency
  • Identity
  • Citizenship or Legal residence in the U.S. (Citizenship/Legal Residency not required for Restricted Medi-Cal)
  • Social Security Number
  • Other Health Insurance Coverage
  • Income
  • Property
  • Disability
