IHSS Independent Providers Enrollment Instructions
Please read the instructions on this page carefully. The IHSS Independent Provider Enrollment Center website is for caregivers or providers of the In Home Supportive Services (IHSS) program in Santa Cruz County.
State laws require that all IHSS providers go through an enrollment process and pass a background check before they are eligible to be paid by the IHSS program.

Read all these instructions first, then use this link to the new IHSS Independent Provider Enrollment Center website to start the enrollment process, all at your convenience!:
- Watch the mandatory enrollment video
- Review the provider enrollment form (SOC 426) and agreement (SOC846)
- Book an appointment date and time to visit our enrollment office location
Go To Your Appointment
Arrive for your appointment on the date you have chosen and remember to bring the following:
- Original Social Security Card – we can not accept a copy.
- Current valid driver’s license or another US government issued photo ID.
- Name on both the Social Security Card and ID must match.
- You will be asked to reschedule your appointment if you do not bring these items with you to your appointment.
Submit to LiveScan Fingerprint Imaging
- At your appointment Public Authority staff will provide you with the LiveScan form you will need to take to one of six (6) locations listed on the Live Scan Fingerprint Locations page.
- After you have read these instructions use this link to return to the IHSS Independent Provider Enrollment Center to continue with your enrollment process.
