What information do I need?
The Adult Protective Services screening social worker will request the following information:
- Your name (or you may request anonymity if you are not mandated)
- The elder or dependent adult’s name, address, telephone, age, and current location
- A description of the circumstances which lead you to believe abuse/neglect is occurring
- Any other information you may have regarding the name of the individual believed to be responsible for the abuse/neglect
- Date, time and place of incident
- Any statement made by the victim regarding abuse/neglect
Results of investigations are kept confidential.
